Purchase Agreement

You, the buyer, must agree to these terms to purchase services. The buyer is the person paying for the testing services. The buyer does not have to be the same person as the patient. 

Purchase Terms

• I agree that I have the patient's permission to order lab tests for them. 
• The patient must read and agree to the Patient Consent terms before I order tests. 
• I agree that I am responsible for paying for sample collection and lab testing services now. I understand that these services will be delivered in the future. 
• Your order is good for 6 months. During this time, it can be used or cancelled. See our Refund Policy for more information. 

Order Fulfillment Terms

• Order fulfillment begins when Patient Choice Labs collects any test sample from the patient. 
• Order fulfillment is complete when Patient Choice Labs releases test results to the patient's requested form of communication. 
• The patient is responsible for providing a form of communication that Patient Choice Labs can reach.
• Orders are still complete if Patient Choice Labs makes a reasonable effort to reach the patient but cannot reach them. 
• The patient can still request a copy of their results if Patient Choice Labs cannot reach them. The patient has the right to update their contact information and request a copy of their results under HIPAA. The patient must prove their identity to do this.
