Patient Consent

You, the patient (or the patient's guardian or legal representative), must agree to these terms to receive services. The patient is the person who will have test samples collected and sent for lab testing. The patient does not have to be the same person as the buyer paying for services. 

Consent for Testing

• I agree that I am voluntarily providing samples (blood, urine, or other) for lab tests. 
• I agree to provide my own samples for testing. 
• I agree to provide an unadulterated sample for testing. 
• I authorize Patient Choice Labs to release test results to the contact information that I will provide when I visit the collection site. 
• If I have a doctor's order, I authorize Patient Choice Labs to release test results to the ordering doctor or other health care provider. 

Notice of Privacy Practices

• I understand that HIPAA, a federal health care law, gives me certain rights to medical privacy and access to my medical information.
• I have received and reviewed the Patient Choice Labs Notice of Privacy Practices.
